Community-based Management of Acute Malnutrition (CMAM) approach is widely recognized for its effectiveness in treating Moderate Acute Malnutrition (MAM) and Severe Acute Malnutrition (SAM) in children under 5 years. Found potent across different socio-economic and geographical settings, CMAM helps volunteers in identifying the target population and initiating treatment for children with acute malnutrition before they fall seriously ill and require medical attention. This approach assumes importance in the backdrop of a facility-based model having limited scope in terms of coverage and impact. CMAM comes in handy especially if there are aggravating factors present in the community such as epidemics, high death rate, food insecurity and a general lack of health infrastructure. Multiple pieces of evidence suggest that children with uncomplicated SAM can be treated effectively in their community setup without being admitted to a health facility.

Ready to Use Therapeutic Foods (RUTF) or locally prepared nutrient-dense foods serve as an indispensable component to treat SAM-afflicted children in their community environs and facilitate earlier recognition of SAM and the timely onset of treatment. In the case of uncomplicated SAM, a strong community component could go a long way in ensuring appropriate care through sensitization and mobilization. Recognizing the magnitude of the malnutrition problem, and the kind of multi-level interventions required to provide a sustainable and holistic solution, Government of India tagged September 2019 as “PoshanMaah”, which translates to “Nutrition Month”. This welcome move underlines the concern that it is time malnutrition is confronted on a war footing by implementing a multi-pronged approach for its redressal.

The target population of this initiative are children up to 6 years of age, adolescent girls and lactating mothers, with the initiative covering all states and UTs in a phased manner. There are specific targets for the reduction of low birth weight babies, stunted growth, incidences of Severe Acute Malnourishment (SAM) and anaemia. Information and Communication campaigns aimed at nutrition awareness at grassroots level forms an integral part of the programme, conducted with inclusive participation of stakeholders at various levels.

The magnanimity of the malnutrition issue cannot be handled effectively by depending only on overarching government schemes, institutions and their limited capabilities. With the proportion of SAM-afflicted children much more than institutional facilities available to treat them, the Community-based Management of Acute Malnutrition (CMAM) approach comes across as a panacea to treat SAM and MAM children in community settings itself. This approach is indispensable to change the current trajectory of malnutrition in India for good. Malnutrition is not new to Indian society. Reeling with acute and severe malnutrition since independence, the National Family Health Survey (NFHS-4, 2015-16) shows a continually grim picture. To treat this deep-entrenched, vicious problem of malnutrition, holistic solutions are an absolute necessity, which needs broad thinking rather than micro-management. This necessitates a deep understanding of SAM, its magnitude, available treatment methodologies and their positive externalities. SAM is a medical condition which cannot be treated with the mere provision of food. It requires proper end-to-end management of RUTF - streamlining the preparation, distribution and use of accessible RUTF to treat SAM and MAM. RUTF provides an aggregate set of nutrients and caloric density for malnourished children. When prepared following the WHO and UNICEF guidelines, RUTF has been medically proven to be effective in decreasing incidence of SAM, quicker and cost-effective than traditional methodologies. Several pilot studies in India across varied settings is also a case in point to reinforce RUTF’s effectiveness. A concerted effort is thus required to close out existing knowledge gaps in nutrition space so that the scourge of malnutrition can be handled in an effective, accessible and sustainable manner using RUTFs.

How therapeutic food can be the game-changer in malnutrition treatment in the Indian scene?

That India’s healthcare system is in dire need of a vital upgrade is a no-brainer. For years, the institutions and stakeholders have toiled hard to formulate the magic formula which could redress the never-ending problem of Severe Acute Malnutrition (SAM) prevalent among Indian children. Though the existing healthcare interventions have been in place and working in tandem with multiple stakeholders, the jury is still out on their effectiveness to deal with SAM. The issue of micronutrient deficiency has been prevalent and known, but not satisfactorily tended to by the institutional healthcare system. One of the crucial reasons for widespread U-5 malnutrition is attributed to inequitable access to balanced nutrition sources. These then make the group vulnerable to different infections and diseases, thus trapping them in a vicious cycle of malnutrition and ailments[3].

Extraordinary scenarios like these - 1 in every child in India is afflicted with severe wasting - requires cost-effective, yet implementable solutions, which can be scaled according to tailored requirements of each state/region[2]. A right step in this direction is the provisioning of various therapeutic food options like Ready-To-Use packages (RUTF). These energy-dense formulations are often at the core of nourishing the children, since they are fortified with critical macro- and micro-nutrients like vitamins and minerals, in addition to proteins and carbohydrates. Considering there is always a threat of water contamination and bacterial infections, RUTF comes in handy as it need not be mixed with water and can be taken without further preparation. Since the scourge of SAM has a history of afflicting everybody irrespective of his/her socio-economic background, the relevance and usage of RUTF need to be seen and appreciated in a much wider context. Given that Indian subcontinent has a variety of climatic zones, RUTF is often climate resistant and can be kept without refrigeration for up to 4 months. This safety factor is essential to gain wider acceptability of RUTF in the Indian context that often becomes difficult, considering the demographics and health parameters change from one region to another.

Since institutional care of SAM children may not always be feasible, especially in rural settings, home-based care is recommended by healthcare experts the world over. RUTF being energy and protein intensive becomes an essential ingredient during a child’s rehabilitation phase. Using RUTF, it was found that there were fewer relapse cases, better recovery, a higher rate of weight gain
and hence reduced mortality rates. It is predominantly clear if the RUTF satisfies the daily nutritional requirements of the child[1].

There have been some genuine concerns of over-reliance on RUTF as a primary source of nutrition. Also, provisioning RUTF on a large scale may require a dedicated funding mechanism rather than banking on purchase by the afflicted population. Such concerns notwithstanding, it is time therapeutic foods like RUTF are implemented at a larger scale, taking heart from the positive, encouraging results derived from implementation in certain Indian states. Time is ripe for taking this exercise to the next level, to garner the “low-hanging” fruits, and handle malnutrition effectively.
